Private Insurance

Many private insurance companies offer home health care services as a benefit. We will check with your carrier to determine exactly what benefits are included in your policy.


Medicaid is the medical assistance program for low income people of all ages who are unable to pay for care and meet the eligibility guidelines. It is financed by federal, state and local funds, and the services covered are set by federal and state law. 

Long Term Home Health Care Program (LTHHCPs)

Also known as Nursing Home Without Walls, the Long Term Home Health Care program operates under a specific Federal Medicaid waiver. It may be part of a CHHA, or be nursing-home based. The program offers health care and support services to the disabled and chronically ill who are medically eligible for admission to a nursing home, but who choose to be maintained at home. Community has contracts with several LTHHCPs that service clients with personal care aides. 

Managed Long Term Care (MLTC)

MLTC Provides health and long-term care services to adults with chronic illness or disabilities, to better address their needs and to prevent or delay nursing home placement. Services include – but are not limited to – nursing, physical therapy, occupational therapy, speech pathology, medical equipment and supplies, podiatry, dentistry, optometry, respiration therapy, transportation and social day care. This program is available to Medicaid recipients who are eligible for admission to nursing homes. CHC has contracts with several MLTC’s that service the clients with personal care aides.
